Questions for the employee:

1. How well did you meet the requirements outlined in your current job description during this review period? Note any particular strengths and challenges.

2. Were there any changes in your job duties that are not reflected in your current written job description? If so, list them.

3. List specific changes or areas of change you intend in order to bring your performance in line with the updated job description.

4. Did you meet the professional/personal goals you set at the last review? Are there any goals to carry over to the next review period? If so, describe.

5. List the professional/personal development goals you would like to achieve for the next review period. Using information from this form and your reviewer's form, you will complete the separate goal sheet during your review meeting. Be prepared. You will be noting your commitment to work toward these goals at your meeting.

4. Did this employee meet the professional/personal goals set at the last review? Are there any goals to carry over to the next review period? If so, describe.

5. List the professional/personal development goals you would like to see this person achieve for the next review period. Using information from this form and the individual's own form, you will complete the separate goal sheet during your review meeting. Be prepared. You will be noting your commitment to the employee for achieving his or her goals.

Questions for peer and other reviewers:

1. Please describe how this individual's job performance has met the responsibilities outlined in his or her job description. Note specifics related to the work connected to your own duties.

2. Please note any special comments regarding this employee's performance. Include appropriate suggestions, commendations and challenges.
